These look like they are coming down the line and right now, it is not clear what the requirements might be. Some are asking for OWNER, but I’d expect to eventually clarify as “authorized person”
ALSO, if someone signs and you have reason to believe that they DO NOT have the authority, this will not protect you.
TBH, this will probably mean the shoots end up in porn studios and with porn friendly home owners. Goodbye to random hotels and serviced apartments.

“I am” is optional, but does go to show due diligence should anything blow up. If you include it, in the UK suggest allowing the following options :-
The Owner.
The Owners Agent/Manager or other representative authorised to grant permission. (1)
The Tenant, this is my home, and I am allowed to use the property for this purpose (2)
The Tenant, this is a property I rent for my business, and I am allowed to used the property for this purpose. (3)
The Tenant’s Agent/Manager, or other representative authorised to grant permission. (4)
1 – Only usually where the property is a regular porn location, managed as such but the owner doesn’t do the day to day.
2 – In the UK tenants have the right to quiet enjoyment of their home, which means they can use it as if they owned it. HOWEVER, running a business requires written permission from the landlord. It’s not clear if an occasional porn shoot is “running a business”. As a producer, I’d ensure that the property couldn’t be identified, just to be safe. If it’s the landlords furniture, stick to bedrooms.
3 – As (1) but the usual set up for a studio in commercial premises. Also most commercial property such as farms, gyms, garages, where the business is just renting, and the business is entirely responsible for the look.
4 – As (3) if the main person has delegated that to the day manager.

No matter what personal problems it might arise if family/friends/employers/media find out about and use it against me at some stage in the future, neither the producers nor any subsequent owner/distributor/licensee will be held liable.
I hereby give to ………………………………….his/her heirs, legal representatives and assigns, those for whom …………………………………. is acting, and those acting with his/her authority and permission:
a) the unrestricted right and permission to copyright and use, re-use, publish, and republish video or pictures of the property/location intact or in part, composite or distorted in character or form, without restriction as to changes or transformations in conjunction with the location name, or reproduction hereof in color or otherwise, made through any and all media now or hereafter known for illustration, art, promotion, advertising, trade, or any other purpose whatsoever.
b) I also permit the use of any printed material in connection therewith.
c) I hereby relinquish any right that I may have to examine or approve the completed product or products or the advertising copy or printed matter that may be used in conjunction therewith or the use to which it may be applied.
d) I hereby release, discharge and agree to save harmless the producers, his/her heirs, legal representatives or assigns, and all persons functioning under his/her permission or authority, or those for whom he/she is functioning, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form whether intentional or otherwise, that may occur or be produced in the taking of said picture or in any subsequent processing thereof, as well as any publication thereof, including without limitation any claims for libel or invasion of privacy.
e) I hereby affirm that I am over the age of majority and have the right to contract in my own name and for the location/property. I have read the above authorization, release and agreement, prior to its execution; I fully understand the contents thereof. This agreement shall be binding upon me and my heirs, legal representatives and assigns.
